You'll find all the practical information you need, such as a map and directions to our offices, practice hours, payment policies and more. We also describe our first visit procedures. And, you can save some time by printing out and completing the new patient forms in advance of your appointment.

Associated Otolaryngology of Pennsylvania is committed to providing the
highest level of care available for all ear, nose, and throat disorders
in a state-of-the-art, convenient, and friendly setting.
What to expect
Being well-prepared for your appointment will ensure that the doctor
has all of the needed information to provide the best possible care for
you. It also will help relieve any unnecessary anxiety you may be
feeling. Educate yourself on your symptoms by reviewing the content on
this Web site. Also, take some time to review our staff page and
familiarize yourself with the doctors. We look forward to your first
visit.
Please arrive 15 minutes before your appointment time. If you are unable to keep this appointment, please give us twenty four (24) hours notice. A no-show fee of $50-$150 will be charged to your account if you do not show for your appointment or if you cancel your appointment with less than twenty four (24) hours notice.
Please present your current insurance card at the time of registration. If your insurance requires a referral, please call your primary care physician. Many offices require at least 48 hours notice in order to process a referral. Failure to obtain this referral may require that the appointment be rescheduled. If you have a copay, please be prepared to pay this at the time of registration.
Please remember, if you have already undergone testing, or have already been evaluated by a specialist for the same or similar condition, we prefer that you hand-carry copies of all pertinent medical records with you. This is especially important regarding radiographic studies; including CT scans or MRI scans. Please bring copies of your CT and MRI reports if you have them, but more importantly, please bring copies of the studies, preferably in CD format.
